Insurance

Please inform our appointment secretary of your health insurance coverage at the time you schedule your appointment. If your plan requires a referral or authorization, we ask that you obtain this prior to your scheduled visit.

Payment

You are responsible for payment of your office visit co-pay, coinsurance and/or deductible at the time of service. We accept cash, checks, Visa, and MasterCard.

Your health insurance plan may require that you receive care from a participating (in-network) physician. If we do not participate in your plan, your benefits will probably be reduced causing an increase in your payment responsibility.
