Diabetes Education Program at Hanover Medical Specialists, P.A.
Since 2003, Hanover Medical Specialists have been an American Diabetes Association Recognized diabetes self management education program. This means that our patient education program for patients with diabetes meets all of the national standards for quality diabetes instruction.
Diabetes self-management education involves a “team approach.” Members of the diabetes team include the doctor, the nurse diabetes educator, the nutritionist, and most importantly, you the patient. Understanding of the many ways that you can help to control your blood sugars is vital to living a full and healthy life with diabetes.
Hanover Medical Specialists Endocrinologists require that all new patients with diabetes participate in our Diabetes Education Program prior to seeing one of the endocrinologists or “diabetes specialists.” This is our opportunity to review what you already know and are doing to manage your diabetes, and to provide you with instructions and guidelines that will help us coordinate your diabetes care. Topics covered include: what is diabetes, how it is diagnosed, signs and symptoms of high and low blood sugars and how to treat these, the importance of exercise, medications, home blood sugar monitoring, and realistic guidelines for a healthy meal plan.
Whether you have had diabetes for many years or just recently been diagnosed with diabetes, on-going education about diabetes is important. New strategies for improved blood sugar monitoring, new medications and new approaches for achieving a healthy lifestyle (physical activity and healthy eating) are being developed daily.
